Surveys are often used to evaluate learning activities by obtaining participant feedback and comments. There are many good online survey software packages available today. However, they all involve manual steps for collecting, saving, retrieving and analyzing the data, and reporting the results. We wanted a system that does all this automatically and in real time. So we developed our own Survey Tracking, Analysis and Reporting system.

Our online real-time survey system was built using PHP and MySQL. It has the following features:

• Flexible format: different types of questions – radio buttons, drop-down lists, open-ended comments – can be used to collect a variety of feedback data.  
• Database storage: all responses are stored to a database table; these can be used or exported for analysis, research and reporting purposes.
• Results: scores are calculated in real time and can be reported in both numerical and graphic formats.
